Traverse Health Clinic offers a Care Management Program, consisting of a Nurse Care Manager & Care Team Coordinator that is here for YOU! 

Medicare patients are eligible to Annual Wellness Visits (AWV) with your Care Team Coordinator and medical Provider to create a personalized health plan. 

Under our Care Management Program, you'll work alongside a Nurse Care Manager who will:

 

MEDICATION REVIEWS:

Check your current medications and discuss any recent health changes.

 

GOAL SETTING & ACTION PLAN

Work together to set achievable goals and create a plan that feels comfortable and confident for YOU.

 

ADDRESS CONCERNS

Talk through any health concerns or needs you may have.

 

POST ER/INPATIENT FOLLOW-UP

We make sure you have everything that you need after an ER, Urgent Care, or inpatient stay and confirm your follow-up appointment with your Traverse Health Clinic provider.
